| What the information means |
Your ip address is your internet address, with that, you can explore the world wide web, without it, you cannot.
Your dns/isp is who you registered your ip with, with that be AT&T, Verizon, SBC, or any of the many and i mean MANY other phone companies.
Server's ip is just like yours, except the owner hosts with it instead, your connecting to it, it's not connecting to you, the owner has opened the port (port forwarding) or has the server on the public end of the owner's network.
Server's dns, this is what you used to connect to this server with, with that be an IP, an ISP, or a domain name, if you used "www" before the actual domain, the "www" is hidden.
In turn you can connect to any server by it's IP, ISP, or domain name (if it has one).
And all an IP is is a shorter ISP, so that is why you can connect to a server using it's ISP as well.

| How do servers get my ip information? |
Simple, when you connect to a server it automatically gets it because your computer connects to it, thus your isp gives it to them.
You can ask your ISP to hide your ip address from others and you can also search for ip address hiders on google.
There are many ways you can hide your ip from others (by faking too!), and to help with servers using your ip for unwanted purposes, you can invest in dynamic ip address's, these change *on the fly* anywhere from every 30 seconds (nice!) to twice a year.

| What are "ports"? |
Ports are area's of a computer or server where you can connect.
Ports are similar to boat docks (aka ports).
Instead of docking a boat on the "port", you connect to this "port".
If they're is no server software listening on that port on that computer, you will not be able to connect to the port, and in some applications, this means you could not connect to the server at all.
Ports can also be handled by many different ways of the application, the most common way to display an address with a port is Address:Port, Address being an IP, ISP, or domain and Port being an number higher than 0 (0 and lower are reserved).
If your using Mac OS X, you'll probably also want to know that ports equal to and lower than 1024 are reserved for security purposes, anything higher than 1024 will work.
A list of ports and what protocol they use:
7, Echo (Echo port, sends any data back to you that you sent, not all computers or servers have this sort of thing).
21, FTP (Files and Folders).
23, Telnet (Issue a terminal/DOS command on the server).
25, SMTP (Send Email).
80, HTTP (Website).
125, POP3 (Receive Email).
443, HTTPS (Secure Website, commonly used for form's that supply credit card info and passwords, etc.).
3306, SQL (Queries for data that are in databases).
